Each team member will need to present a signed copy of the JVA Participant Liability Release Form 2010 at the event registration.
Each team member needs to complete the JVA Participant Medical Waiver Form 2010. The Head Coach should have the original form with him/her at all times. The Coach will need to show that they have these forms at the event registration.
